c) Does the company have a physical location?

One of the web sites that I currently find most useful in my business does not list a physical address on their web site. But then that already tells me a lot about them, faithful; and efficient as they are. It tells me that they are a young and hungry business just starting out, determined to make an impression. But it also makes me more careful.

d) What will they recommend?

A business that is desperate to sell is never a good sign. Having said that, let me add that there is a big difference between a desperate business and an aggressive business. Both are eager to sell, but for different reasons. A desperate business is eager to sell to pay bills and survive. You do not want to deal with such a company for something as sensitive as your web hosting. Tell-tale signs of desperation are when the lowest rate is quickly recommended without taking into account your needs and requirements. Many times desperation is easy to sense. All you need to do is give your needs briefly and ask what they recommend. Even give information that is just a little short of enough details and see how quickly they recommend something without asking you further questions.
